مركز النادر لعلاج الصقور

يشهد مستشفى الصقور الموضحة بياناته أعلاه أن الطير المذكور أعلاه قد تم فحصه فحصًا سريريًا شاملاً من قبل الأخصائيين في مستشفى النادر البيطري لصقور حفر الباطن، وقد تبيّن أن الطير خالٍ من أي أعراض مرضية ظاهرة أو أمراض وبائية معدية، وذلك حسب الفحص السريري والفحوصات المخبرية اللازمة، وكانت جميع النتائج سليمة في وقت إصدار هذه الشهادة. وتعتبر هذه الشهادة سارية لمدة عشرة (10) أيام فقط من تاريخ الإصدار، وقد أعطيت هذه الشهادة بناءً على طلب مالك الطير دون أدنى مسؤولية قانونية أو طبية على المستشفى عن أي أمراض قد تظهر لاحقًا بعد تاريخ الفحص والإصدار.

This is to certify that the above-mentioned falcon has been clinically examined by the specialists at Al-Nader Falcon Veterinary Hospital in Hafar Al-Batin. At the time of examination, the bird was found to be free from any apparent clinical signs of illness or contagious and epidemic diseases, according to the clinical examination and the necessary laboratory tests carried out. All results were found to be within normal limits at the date of issuing this certificate. This certificate is valid for ten (10) days only from the date of issuance and is granted at the request of the owner of the falcon without any legal or medical responsibility on the hospital for any diseases or conditions that may appear after the date of examination and issuance of this certificate.
